Porting Documents
If you are moving an existing number to Concert Telecommunications, you must complete a Letter of Authorization (“LOA”). There are two versions: one for local telephone numbers and one for toll-free numbers.
You may also complete the associated PDF forms, sign them online or on paper, and email them to operations@concerttelecom.com or fax them to 984 375-4709.
